What To Bring With You To Your First Appointment
For your first appointment please bring a valid drivers license or state ID, your medical insurance card, and all new patient paperwork. If you are being seen for a specific condition or being reffered to us from a different provider please bring your medical records and any recent laboratory work.
*If you have not pre-completed the new patient paperwork please arrive 15-20 minutes early to your appointment, thank you!
